1. Discharge Patient
Medical Review The medical team will review your condition and treatment progress to determine if you are ready for discharge.
Discharge Criteria Once you meet the discharge criteria, a discharge plan will be created, which includes instructions for follow-up care, medications, and any lifestyle changes needed.
2. Preparation for Discharge
Discharge Summary A discharge summary will be prepared, detailing your hospital stay, diagnosis, treatments received, and instructions for follow-up care.
Medication Instructions The medical team will provide you with prescriptions for any medications you need to take after leaving the hospital, along with instructions on how and when to take them.
Follow-Up Appointments Schedule any necessary follow-up appointments with your healthcare provider or specialists.
3. Patient Instructions
Care Instructions Review and follow the discharge instructions provided by your healthcare team. This includes guidance on wound care, activity restrictions, diet, and medication management.
Emergency Contact Know whom to contact if you experience any complications or have questions after discharge.
Follow-Up Care Attend any scheduled follow-up appointments to monitor your recovery and address any ongoing medical needs.
Patient Feedback You can provide feedback on your hospital experience through a feedback form or online portal, which helps us improve our services.
